Medication Administration Record
The medication administration record (MAR) is a medication form for a student that is prescribed medicine. When you complete the document, please return the form to the nurse at your student's school. Click here to access the MAR form.

Over-The Counter Medication for Field Trips
The form is for authorization for any over-the-counter medication for an overnight field trip. The form must be completed in its entirety and signed by a physician and a parent. The medication must be delivered to the school by an adult, in the original sealed container, no later than one week before the trip. Click here to access the over-the-counter form.

Self-Carry Medication Form
It is the policy of North Ridgeville City Schools to prohibit students from possessing or using prescriptions or over-the-counter medication on school buses or school property. However, because of severe medical conditions, a student may need to carry a rescue inhaler for asthma, an epinephrine autoinjector for severe allergic reactions, or other life-saving medication.
If the health care provider feels that your student must carry life-saving medication, please have them complete it and sign the form stating the medical necessity for carrying the medication. The parent must also sign the form.
