Medication Administration Record
The medication administration record (MAR) is a medication form for a student that is prescribed medicine. When you complete the document, please return the form to the nurse at your student's school. Click here to access the MAR form.

Over-The-Counter Medication
The form is for authorization for any over-the-counter medication as long as the instructions are the same as listed on the medicine packaging. This form requires a parent or guardian's signature. The medication must be delivered to the school by an adult in the original sealed container. For further guidance, please review the Medication Administration Guidance for NRCS. Self-Carry Medication Form
It is the policy of North Ridgeville City Schools to prohibit students from possessing or using prescriptions or over-the-counter medication on school buses or school property. However, because of severe medical conditions, a student may need to carry a rescue inhaler for asthma, an epinephrine autoinjector for severe allergic reactions, or other life-saving medication.
If the health care provider feels that your student must carry life-saving medication, please have them complete this form and sign it stating the medical necessity for carrying the medication.The parent must also sign the form.
